Cholera can kill a healthy adult in under half a day through dehydration alone — and has been reliably curable since the 1970s with a solution of clean water, sugar, and salt that costs pennies. It's also the disease that founded modern epidemiology, decades before anyone knew what caused it. We are currently living through its seventh pandemic, which began in 1961 and has never officially ended.

A little history

The map that broke a pandemic — and a bad idea.

The miasma theory, disproven by a map.

In 1854, most doctors believed cholera spread through "bad air." John Snow didn't. During a lethal outbreak in London's Soho district, he interviewed residents and mapped every death by address, tracing the cluster to a single water pump on Broad Street. He had the pump handle removed; the outbreak was already fading, but his map — dots of death clustered tightly around a well — became one of the founding acts of modern epidemiology, decades before germ theory was widely accepted.

The germ, found twice.

Italian anatomist Filippo Pacini identified the cholera bacterium under a microscope that same year, 1854 — and was ignored for decades. Robert Koch is far more commonly credited with discovering it, in 1883, during an outbreak in Egypt. The bacterium wasn't formally renamed to credit Pacini's overlooked work until 1965, more than a century after he first saw it.

Cholera doesn't damage the body directly so much as drain it — a patient can lose more than a tenth of their body weight in fluid within hours. Left untreated, that climb in fluid loss tracks almost exactly with a climb in fatality. Treated in time, that same climb barely matters. Hover a step to read it.

Introduced, not inherited · Haiti, 2010

The outbreak a country didn't have until peacekeepers arrived.

Haiti had been cholera-free for over a century before October 2010 — ten months after a devastating earthquake had already killed more than 200,000 people and left much of the country's infrastructure in ruins. A cluster of severe diarrheal illness appeared along the Artibonite River, near a base housing UN peacekeepers newly arrived from Nepal, where a cholera outbreak was active at the time. Genetic testing later matched the strain to a South Asian lineage nearly identical to Nepal's.

Untreated sewage from the base had been discharged into a tributary that hundreds of thousands of Haitians used for drinking, bathing, and washing. The outbreak went on to infect roughly 820,000 people and kill nearly 10,000 over the following decade — a toll layered on top of the earthquake's own. The United Nations denied legal responsibility for years, citing diplomatic immunity, before its then Secretary-General issued a formal apology in 2016 — without an accompanying compensation fund for victims.

What keeps it in its seventh pandemic isn't the bacterium; it's every collapsed sewage system, every under-supplied vaccine stockpile, and every government that treats clean water as optional. The map that founded epidemiology also proved the fix was never medical. It was plumbing.
