broke the world

Before antibiotics, before germ theory, before microscopes, infectious diseases regularly killed a third of a continent's population in a single event. The Black Death alone killed more humans in four years than the First World War killed in four — and left the survivors living in a fundamentally different civilization. This is what we know, and what we finally understand, about the pandemics that shaped history.

A note on the pathogen: The Black Plague was not caused by a virus. It was caused by the bacterium Yersinia pestis, transmitted from infected rodents to humans via flea bites — and, in its pneumonic form, directly between people. This distinction matters because bacteria respond to antibiotics: the same disease that killed 60% of medieval Europeans is now treatable with a course of doxycycline, if caught early.

What it is

Yersinia pestis is a gram-negative bacterium in the family Yersiniaceae. It evolved from Yersinia pseudotuberculosis, a gut pathogen, through a small number of genetic mutations that gave it the ability to colonise fleas, spread to the lymphatic system, and overwhelm the immune response — changes that happened within the last 5,000 to 20,000 years on an evolutionary timescale.

In nature the bacterium circulates silently within populations of wild rodents, causing them little harm. Transmission to humans happens through the bite of infected fleas, through contact with infected animal tissue, or — in the pneumonic form — directly between people through respiratory droplets. All three forms are the same bacterium, in different anatomical locations.

How it was confirmed — ancient DNA

The identification of Y. pestis as the cause of the Black Death was debated for over a century — some historians argued for anthrax, viral hemorrhagic fever, or even an unknown disease. The answer came from ancient DNA (aDNA) extracted from the teeth of burial victims.

By the 1990s and 2000s, aDNA from plague pits in France, Germany, and England's East Smithfield mass burial confirmed the bacterium. The 2022 Nature study went further — DNA from seven individuals at Kara-Djigach and Burana cemeteries in Kyrgyzstan, buried with tombstones dated 1338–1339 and inscribed "pestilence," confirmed those graves hold the direct ancestral strain of the Black Death: the "Big Bang" of Y. pestis diversity that seeded all the major lineages that swept the medieval world.

Bubonic plague
Flea bite → bacteria enter lymphatic system → lymph nodes swell into painful swellings called "buboes" (Greek: groin). The most common form. Named the Black Death for the dark discolouration of necrotic skin.

Septicemic plague
Bacteria enter the bloodstream directly, overwhelming it before any bubo can form. Rapid organ failure, internal bleeding, and black skin discolouration (acral gangrene) — the other explanation for the "Black" in Black Death. Can arise from flea bite or develop from bubonic.

Pneumonic plague
Bacteria infect the lungs — either from inhaling droplets from another person, or from bubonic/septicemic plague spreading to the lungs. The only form that spreads person to person, which is what makes it theoretically the most dangerous and what powered super-spreader events in medieval hospitals and cities.

How the Black Death changed the world
1350s onward

End of feudal labour
A third to a half of the agricultural workforce was dead. Suddenly, surviving peasants could negotiate wages or simply leave — something inconceivable before. The feudal system that had organized European society for centuries never fully recovered.
The Church undermined
The clergy died at the same rate as everyone else — despite their prayers. The inability of the Church to explain or prevent the catastrophe permanently fractured its authority over European intellectual life, accelerating the conditions for the Renaissance and later the Reformation.

First public health institutions
Italian city-states invented the formal public health apparatus: Venice created the world's first quarantine in 1377 (40-day isolation, "quarantina"). Milan built the first permanent board of health in 1450. These institutions are the direct ancestors of today's CDC and WHO.

Death in art and culture
The danse macabre, memento mori, and a profound preoccupation with death entered European art and literature for the next two centuries. Boccaccio's Decameron, Petrarch's elegies, and later Chaucer's Canterbury Tales were all direct responses to the plague experience.

Jewish communities massacred
Across Germany, France, and the Holy Roman Empire, Jewish communities were blamed for poisoning wells and massacred in their hundreds of thousands — one of the worst pre-Holocaust persecutions in European history. Pope Clement VI issued two papal bulls stating Jews were dying of plague too and were not responsible, but it did little to stop the violence.

English language emerged
The Norman French-speaking ruling class died at similar rates to English-speaking commoners. Within a generation, the social pressure to speak French had collapsed, and English began its rise as the language of commerce, law, and eventually empire — a direct if unintended consequence of the plague's social levelling.

Wages and trade shifted
A Europe with a third fewer people needed a third fewer things but still had the same capital — land, ships, tools, buildings. This compression of the labour-to-capital ratio, combined with the trade disruptions, is one of the structural reasons economic historians link the Black Death to the long-run rise of mercantile capitalism in the 15th and 16th centuries.

Accelerated the Renaissance
Florence lost roughly 60% of its population. The survivors inherited concentrated wealth, disrupted hierarchies, and a visceral awareness of human fragility. The Renaissance's individualism, its questioning of authority, its fascination with the body — each one traces at least partial roots to the civilizational rupture of 1348.

Why it still circulates

Yersinia pestis is not a human disease that could be eradicated the way smallpox was. It lives in wild rodent populations — ground squirrels, prairie dogs, and chipmunks in the American Southwest; gerbils and rats across Central Asia and Africa — where it causes little harm and has persisted for millennia. As long as those reservoir populations exist, the bacterium does too. Modern rodent die-offs in plague-endemic zones are often the early warning that transmission to humans may follow.

In the United States, plague cases cluster in New Mexico, Arizona, Colorado, and California — the descendants of a Third Pandemic introduction around 1900 that spread into the Western rodent population and has never been eradicated. Cases are typically traceable to contact with infected animals or their fleas during outdoor activities.

Treatment — what changed everything

Streptomycin, the first effective treatment, was identified by Selman Waksman in 1943 and is still used today. Doxycycline and ciprofloxacin are the current first-line choices. If treatment begins within 24 hours of symptom onset for pneumonic plague, or within a few days for bubonic, survival rates exceed 90%. The same disease that swept through medieval Europe killing 6 in 10 is now, in the context of access to antibiotics and rapid diagnosis, essentially curable.

No vaccine is currently licensed for general use in most countries, though candidates exist. The US discontinued its commercial vaccine in 1999 due to limited demand. The plague's continued presence in the world is a story about ecology, not medical inability — we simply cannot reach all the rodent reservoirs.
