Before we called it tuberculosis, we called it consumption — and it consumed everything: our cities, our art, our medicine, our dead. It shaped how we build, breathe, and grieve. Two centuries on, it is once again the deadliest infectious disease on Earth — and we have had a cure since 1943.

A little history

The disease that made us modern.

It was beautiful, once.

For a century, consumption was the fashionable way to die. The Romantics made a virtue of the wasting look — pale skin, bright eyes, a thin frame, a flush of fever. Keats coughed blood at twenty-five and called it his “death-warrant.” It took the Brontë sisters, Chopin, Chekhov, Kafka, and Orwell. Poets envied it; corsets imitated it.

Then it built the modern world.

The fight against it reshaped everything. Sanatoriums pushed us toward sunlight, fresh air, and ventilation. Fear of the “spit” invented public-health campaigns, sputum tests, and the very idea of the contagious crowd. It helped create the sanitarium, the X-ray clinic, and the notion that the state should manage disease. Consumption, in the end, consumed the old world and set the terms of the new one.

Antiquity
Phthisis

TB scars appear in Egyptian mummies and Neolithic bone. Hippocrates names phthisis — “wasting” — the most common fatal disease of his age.

1720
“Living creatures”

English physician Benjamin Marten proposes consumption is caused by minute, invisible creatures passed between people — a germ theory more than a century ahead of proof.

The stethoscope

René Laennec, studying the disease that would kill him, invents the stethoscope to hear the lungs and names the tubercle.

1800s
The White Plague

Industrial cities pack people into tenements; TB kills perhaps one in four Europeans. It is romanticised as the “robber of youth.”

1865
Proven contagious

Jean-Antoine Villemin infects rabbits with matter from human TB lesions, proving experimentally what Marten had only guessed: consumption spreads.

1882
Koch finds the bacillus

Robert Koch identifies Mycobacterium tuberculosis — proof that the beautiful death is a common germ, spread on the air.

1921
BCG

The BCG vaccine debuts — still the only licensed TB vaccine a century later, and only partly protective.

1943
The first cure

Streptomycin, discovered in a New Jersey soil sample, becomes the first antibiotic to beat TB. For the first time, consumption is curable.

1952
Isoniazid

A cheap, oral drug becomes the backbone of TB treatment worldwide — and, taken alone, the first step toward resistance.

1993
Global emergency

WHO declares tuberculosis a global health emergency — the first disease ever given that designation.

1990s
DOTS

WHO rolls out directly-observed therapy to hold the six-month drug course together — and to keep resistance from breeding.

2010
Rapid testing

GeneXpert molecular testing cuts TB and drug-resistance diagnosis from weeks of culturing to under two hours.

2011
Mumbai: the drugs stop working

Clinicians in Mumbai report tuberculosis resistant to every drug they can test — the disease we had beaten, unbeaten again.

2012
Bedaquiline

The first new TB drug in over forty years is approved, built for the multidrug-resistant strains streptomycin’s heirs could no longer touch.

2020s
New weapons

The all-oral BPaLM regimen shortens treatment from two years to six months. A new vaccine, M72, enters phase-3 trials — the first candidate in a century.

When the staircase runs out.

In late 2011, Dr. Zarir Udwadia and colleagues at Mumbai’s Hinduja Hospital described a handful of patients whose tuberculosis was resistant to every drug they could test — first-line, second-line, all of it. The press called it totally drug-resistant TB. The World Health Organization declined to make the label official, in part because no lab tests every drug — but the meaning was plain: in one of the world’s densest cities, TB had walked back off the bottom step of the staircase.

It was not a freak. It was manufactured — by broken drug supply, unregulated private prescribing, months-long treatments abandoned halfway, and the crowding that lets one cough seed a hundred lungs. Resistance is less a mutation than a mirror.

The reckoning

Curable since 1943. Still number one.

A person dies of tuberculosis roughly every twenty-five seconds — of a disease we learned to cure eighty years ago, with drugs that cost pennies. TB is not a mystery of biology anymore. It is a measure of who we decide is worth saving. Consumption created the modern world; whether the modern world finishes it is a choice, not a discovery.
