For a generation, measles was a disease the United States had beaten. The country declared it eliminated in 2000 — no continuous, home-grown transmission. Twenty-five years later it is spreading again. In 2025 the U.S. logged 2,289 confirmed cases, the most in any year since 1992, and its first measles deaths in a decade. The virus did not change. Our defenses did: childhood vaccination has slipped below the level needed to keep measles from catching.

The trend line

A quarter-century of near-zero — then a wall.

After elimination, the U.S. averaged roughly 180 cases a year, almost all imported by unvaccinated travelers and quickly contained. Import sparks that once fizzled now find kindling: pockets of under-vaccinated communities where one case becomes hundreds. The 2014 and 2019 spikes were warnings. 2025 broke the scale.

Where it spread

One county lit the match.

The 2025 resurgence had an epicenter: Gaines County, Texas, an under-vaccinated rural community where the outbreak began in late January and grew to hundreds of cases — the large majority of the year's national total. From there it jumped to New Mexico, Kansas, Oklahoma and Ohio, and single cases surfaced in more than 30 states as travelers carried it across the country.

The through-line

Nearly every 2025 cluster traced back to the same root: communities where MMR coverage had fallen below the 95% herd-immunity threshold. Where vaccination held, imported cases stopped at one or two.

The cause

This is a vaccination story.

Measles is among the most contagious diseases known — one case can infect up to 18 others. That is why the bar for community protection is so high: about 95% of people must be immune to stop it spreading. Two MMR doses are ~97% effective. But kindergarten MMR coverage has fallen every year since the pandemic, dipping to 92.5% in 2024–25, while vaccine exemptions hit a record high. The gap between 95% and where we are is exactly the room measles needs.

Of every 100 measles cases in 2025, roughly 93 were people who were not vaccinated — or didn't know if they were. Measles isn't back because it got stronger. It's back because immunity got thinner.

Late January 2025
An outbreak begins in Gaines County, West Texas, a rural community with low vaccination uptake. It spreads through unvaccinated children and families.
February–March 2025
The cluster crosses into New Mexico, then Kansas and Oklahoma. The U.S. records its first measles deaths in a decade — two unvaccinated children in Texas and an unvaccinated adult in New Mexico.
Spring 2025
By mid-April, 27+ states report at least one case; new outbreaks flare in Ohio. Most cases are travel-linked sparks landing in under-vaccinated pockets.

National cases pass 1,000, then keep climbing — the worst year since the 1990s and a direct threat to the elimination status the U.S. has held since 2000.
Year-end 2025
2,289 confirmed cases across 48 outbreaks. The West Texas outbreak is declared over, but the pattern is set — and 2026 opens with new clusters already reported.
